Report a workplace health and safety concern
Tell HSE about an unsafe work, public-service, recognised civic-work or duty condition within its remit.
Use this service where you believe an organisation, service, public body, department, local authority or other responsible body is not managing a workplace or duty safety risk appropriately.
The native HSE concern form records the organisation and activity, hazard, possible harm, people at risk, existing controls, incidents or near misses, evidence, action sought and any urgent safeguarding or safety issue.
Submitting a concern does not automatically open an inspection case or prove a breach. HSE validates and triages the concern before deciding the appropriate regulatory response.
